Immunization Records

Current, up-to-date vaccination records as required by state childcare licensing regulations.

Must be signed by a licensed healthcare provider
Must reflect all age-appropriate vaccinations
Must be updated any time new vaccines are received

Emergency Contact Form

A completed form listing all authorized pickup adults and emergency contacts for your child.

List at least two emergency contacts beyond parents
Include all adults authorized for daily pickup with Identification
Must be re-submitted if contacts change at any time

Identification Documents

A copy of your child's birth certificate or government-issued ID confirming identity and date of birth.

Birth certificate preferred — passport accepted
Required once at initial enrollment consultation

Required if your child needs any medication — prescription or over-the-counter — administered at the center.

Must be completed for each medication separately

A physician-signed action plan for children with known allergies, asthma, or other health conditions requiring staff awareness.

Required for any diagnosed allergy or chronic condition
Must be signed by the child's physician and parent
Reviewed with lead teacher before child's first day
